膝关节股骨假体的个性化设计与匹配程度分析

Personalized Design Method and Matching Analysis of Femoral Knee Prosthesis

  • 摘要: 为了验证个性化设计方法在股骨假体匹配性上的合理性,并为股骨假体的个性化设计提供参考,提出以截骨面覆盖率和假体重合度为指标来分析股骨假体匹配性的方法.首先参考胫骨平台覆盖率的概念给出股骨假体截骨面覆盖率的定义,并引入假体重合度来分别量化股骨假体在截骨面和外形上的匹配程度;然后基于CT扫描数据重建人体膝关节三维模型,提取模型表面特征并完成股骨假体的个性化设计.将个性化股骨假体与Nex Gen,GEMINI和LEGION这3种标准型股骨假体一起进行模拟植入实验,测量并计算4种股骨假体截骨面覆盖率和假体重合度的值,结果表明,个性化股骨假体在截骨面和外形上的匹配性均优于3种标准型股骨假体;而截骨面覆盖率和假体重合度能够对假体匹配性进行系统的分析和评价,其结果可以作为膝关节假体个性化设计的重要参考.

     

    Abstract: In order to verify the rationality of personalized design in femoral prosthesis matching performance,and provide a reference for the personalized design method of femoral prosthesis, this paper presents amethod for the analysis of femoral prosthesis matching by using the osteotomy coverage rate and prosthesiscoincidence degree. First, this study defines the osteotomy coverage rate of femoral prosthesis based on theconcept of tibial plateau coverage rate, and introduces the concept of prosthesis coincidence degree to quantifythe matching degree of the femoral prosthesis on the osteotomy plane and the shape respectively. Thenthe 3D model of knee joint is reconstructed based on CT scanning data, and a personalized femoral prosthesisis designed based on the surface feature which was extracted from the model. The personalized prosthesiswas implanted by computer simulation with the other three types of femoral prosthesis of NexGen, GEMINIand LEGION, and the values of osteotomy coverage rate and prosthesis coincidence degree of the four typefemoral prostheses are calculated in the end. The results show that the matching performance of the personalizedfemoral prosthesis is better than the standard prostheses on the osteotomy surface and the shape of prosthesis, and the indexes of osteotomy coverage rate and prosthesis coincidence degree can be used toanalyze and evaluate the prosthesis matching performance systematically. The value of the two indicatorscan be used as an important reference for the design of personalized knee prosthesis.
